FDA Approves Lilly's Inluriyo With Verzenio for ESR1-Mutated Breast Cancer

An all-oral pairing that doubled median progression-free survival to 11.1 months from 5.5 in ESR1-mutated patients in the EMBER-3 trial. It is Inluriyo's second FDA approval in under a year.

On 18 September 2026 the FDA granted full approval to Inluriyo (imlunestrant), an oral estrogen receptor antagonist, with Verzenio (abemaciclib), a CDK4/6 inhibitor, for adults with ER+, HER2-, ESR1-mutated locally advanced or metastatic breast cancer that progressed after at least one line of endocrine therapy, the mutation found by an FDA-authorized test^[1]
In the Phase 3 EMBER-3 trial's ESR1-mutated patients (n=159), the combination doubled median progression-free survival to 11.1 months from 5.5 months on Inluriyo alone (HR 0.53, 95% CI 0.35-0.80)^[1]; objective response was 35% against 15%, out of 874 patients enrolled^[4]
The FDA also approved Guardant Health's Guardant360 CDx blood test as the companion diagnostic; the dose is imlunestrant 400 mg once daily on an empty stomach with abemaciclib 150 mg twice daily^[4][5]
Diarrhea hit 86% of combination patients (Grade 3-4 in 9%) and neutrophil counts fell in 86% (Grade 3-4 in 21%)^[1]; overall survival data were still immature and the benefit came from a subgroup analysis^[3]
It is Inluriyo's second FDA approval in under a year, after its September 2025 monotherapy approval; Lilly says Inluriyo already reaches over half of patients starting an oral SERD^[1]
Rival oral estrogen-receptor drugs include Arvinas and Pfizer's Veppanu (vepdegestrant), approved in May 2026, and AstraZeneca's camizestrant, given accelerated approval in September 2026^[2]
Inluriyo is also in the Phase 3 EMBER-4 adjuvant trial, with more than 8,000 patients and first results expected in 2027^[1]

AstraZeneca's phase 3 trial of BGF MDI 320/14.4/9.6 μG in copd reaches primary completion, the point at which the last participant's primary outcome is measured. The trial enrolled 5,000 participants. This study will evaluate the effect of triple ICS/LAMA/LABA therapy with BGF MDI 320/14.4/9.6 μg on cardiopulmonary outcomes relative to LAMA/LABA therapy with GFF MDI 14.4/9.6 μg in a population with COPD and elevated cardiopulmonary risk.
